On-Chain Health Data: The Gap Between Promise and Reality

The global healthcare data market is estimated at $30 billion, with electronic health records (EHRs) generating petabytes of data annually. Yet the blockchain layer handles less than 0.0001% of that volume. Why? The answer lies in the fundamental tension between transparency and privacy. Blockchain excels at immutable, public record-keeping, but healthcare data requires granular access control, regulatory compliance (HIPAA, GDPR), and the ability to delete or amend in specific cases. Smart contracts alone cannot solve this. Zero-knowledge proofs and off-chain storage solutions (like IPFS or Arweave) add complexity and cost. A typical ZK-proof for a patient record costs $0.50–$2.00 in gas today—prohibitive for mass adoption. Volatility is the tax you pay for illiquid assets. Here, the illiquid asset is patient trust. The low transaction volume is not because blockchain can’t handle healthcare data—it’s because the incentives are misaligned. Hospitals and insurers benefit from data silos. Patients are rarely given true ownership of their records. Regulation punishes data sharing. Until these economic and legal barriers shift, no amount of cryptographic elegance will move the needle.
